National Home Health Services started its initial operations in San Jose, California in 2009 with the mission of providing timely, efficient, and professional skilled nursing and rehabilitative care to Bay Area residents.
As the years progressed, the demand for our services grew so we began to expand throughout Northern California. We currently have nine offices in Northern California and more offices are in development.
As a company, we pride ourselves on focusing on patient-centered care.
What started with only two employees has grown into a mighty force of close to five hundred professionals who cover all aspects of your care.
From simple medication management, complex infusions and post-surgical rehabs to regaining your ability to move and communicate after a life changing event like a stroke or cancer – we cover it all in the most efficient, professional and caring way possible.

Home health services offer a comprehensive solution for your nursing and rehabilitation needs.
The services are offered in your private home or in a facility settings. Whether you are recovering from surgery, stroke, have IV or wound care needs – our goal is to provide you with a full range of quality care in the comfort of your own residence. Almost all services that would normally be available to you in the hospital setting would be provided for you at home.
Getting started is simple and straightforward.
All we would need is a referral from your attending doctor or the doctor in the hospital to get started. You can speak with your case manager in the hospital prior to discharge to help obtain a home health referral.
Once we receive a referral, our intake staff will gather the necessary information about the services needed and obtain all medical orders from your doctors. Within 24 hours from receiving the orders, nurse or therapist will come to you for the assessment.
Once your condition is assessed, we will come up with your personal plan of care, which includes the interventions, goals and frequencies of treatment. You, your family or caregiver will be an integral part in developing your care plan. Once it is done, the care plan will be submitted to your doctor for approval.
Our nurses and therapists will promptly contact you after your discharge from the hospital and organize all aspects of your care.
